β2 microglobulin Antibody (β2-MG Ab)
β2 microglobulin, also known as B2M, is a member of MHC class I molecules. It is present in all nucleated cells, but does not include red blood cells. In humans, β2 microsphere eggs are encoded by the B2M gene.
β2 microglobulin not only binds to the α chain of MHCI molecules, but also binds to MHCI-like molecules, such as CD1 and Qa. Another additional function of β2 microglobulin is to combine with HFE protein to jointly regulate the expression of hepcidin in the liver, and the degradation of iron transport membrane proteins on the plasma membrane of hepcidin intestinal epithelial cells and macrophages, which leads to The iron ions taken up are reduced and the iron ions in the red blood cells are released. This loss of function can lead to excessive iron and hemoglobin deposition. When patients undergo long-term hemodialysis, β2 microglobulin can be polymerized into amyloid fibers and deposited in bones and joints. This disease is called dialysis-related amyloidosis.
β2 microglobulin is necessary for the expression of MHC class Ⅰ protein cell surface and the stability of the peptide binding region. In fact, in the absence of β2 microglobulin, MHC class I proteins (whether typical or atypical) are rarely detected on the cell surface. In the absence of MHC class I proteins, CD8 T cells cannot grow, and CD8 T cells are a type of daughter cell among T cells produced during the process of acquired immunity. The low content of β2 microglobulin reflects that HIV has not spread.
In patients with multiple myeloma and lymphoma, β2 microglobulin content will increase, however, in these cases, primary amyloidosis (amyloid light chain) and secondary amyloidosis (amyloid Protein-related proteins) are relatively common. The normal content of β2 microglobulin is less than 2mg / L. However, for multiple myeloma, β2 microglobulin expression may also exceed normal levels. In the diagnosis of multiple myeloma, the detection of β2 microglobulin content is an important indicator. If a patient's β2 microglobulin content is less than 4mg / L, then his expected median survival is 43 months, however, if the β2 microglobulin content is greater than 4mg / L, then his median survival is only 12 month. However, β2 microglobulin is different from the monoclonal immunoglobulin (MGUS) of uncertain significance, and it has a better prognostic effect on the observation of myeloma (low grade).
